He was the son of John Botham Hayes, who died 31 May 1918, and Fanny Hayes. Fanny Hayes was named as Frederick's next of kin when he enlisted; her address was Edward Road, Hill Top, Eastwood. CWGC records his next of kin as his mother, Mrs S Bircumshaw (formerly Hayes) of 8 Edward Road Eastwood Nottinghamshire. His mother remarried, surname 'Saunders', and died 22 May 1933 aged 82. Frederick had a brother, John William Hayes.

Army Service Record survives (RHQ Grenadier Guards archive). He enlisted in Mansfield on 4 February 1915 at the age of 22 years and 90 days. Home Service 4 February 1915-5 November 1915, Western Front 6 November 1915. Appointed Lance Corporal 6 June 1916. In action at Morval and posted missing 25 September 1916, next of kin informed 16 October 1916 but death not confirmed until 5 July 1917. Serre Road Cemetery
Listed in Nottinghamshire County Council register of employees who served ((Nottinghamshire Archives ref CC CL 2 /12/1/1), record completed by John William Hayes (brother). Family gravestone, Eastwood cemetery: 'In loving memory of John Botham, beloved husband of Fanny Hayes, died May 31st 1918 aged 44 years. Also our dear son, Frederick Richard, Grenadier Guards, killed in action at Morval Sept 25th 1916 aged 23 years ‘He did his duty nobly regardless of any cost’. Also Fanny Saunders (above named Fanny Hayes), died 22 May 1933 aged 82 years.'
