Nominate a young person for a 4Uth Award

The Nottinghamshire Outstanding Achievement 4Uth Award is a Nottinghamshire County Council initiative which is being supported by the Nottingham Post to celebrate the achievements of young people across the county.

First prize will be £250 in vouchers of the winner's choice.

Closing date: Friday 26 June 2015

To be nominated, a young person must fulfil a number of criteria.

Does the young person agree to be nominated? »
Does their parent/guardian agree to the nomination and have the young person's details and photograph included in press releases, as well as the young person taking part in media interviews if required? »
Name of the person(s) making the nomination.
Using no more than 500 words tell us about what they have achieved and what barriers they have overcome. If appropriate say something about their commitment and the effort they have put in to make this achievement.
